I just received this letter from ilumed that mentions my doctor/practice. What is this & what am I supposed to do?

A There were 2 letters sent; Annual Notice and Voluntary Enrollment with a form. First, determine which type of letter you received. For example, below your address does it say, “Required Annual Notice” or is there a “confirmation form” and a return envelope with the letter?

A If it is the “Annual Notice”: There is nothing you need to do. This is a courtesy letter letting you know that your doctor signed a contract with ilumed to be part of a new Medicare program to improve the quality of care for their patients at no cost to you. Although nothing changes to your Medicare benefits, you may receive additional benefits. Ilumed also has someone available whenever you need help making appointments. We welcome you calling us at 800-481-8745 to learn more about what will be offered.

A If it is the “Voluntary Enrollment” with the form: This is a courtesy letter letting you know that your doctor signed a contract with ilumed to be a part of a new Medicare program to improve the quality of care for their patients at no cost to you. Your doctor thinks you might benefit from the services offered by ilumed. Although nothing changes to your Medicare benefits, you may receive additional benefits. Ilumed also has someone available whenever you need help making appointments. We welcome you calling us at 800-481-8745 to learn more about what will be offered.

What is ACO REACH?

ACO REACH stands for Accountable Care Organization Realizing Equity, Access and Community Health. We are a company that has contracted with Medicare to improve patient care at no cost to you while potentially offering you added benefits.

Will this affect my existing relationship with my doctor?

No.

What if I only live here half of the year?

You can see any doctor that takes Medicare Fee-For Service. When you are here, you will have access to included benefits and some will be provided telephonically.

Is there any cost to me?

No.

How are my Medicare benefits affected?

It does not change your Medicare benefits. You keep all your current Medicare Benefits and potentially gain additional benefits, such as having someone help you make follow up doctor appointments. We want to help you avoid delays in making appointments.

How do I benefit from the ACO REACH if I have supplemental insurance?

You will also receive the following benefits at no cost to you: $0 co-insurance for your primary care provider, free transportation to and from your doctors visits and pharmacy (where available), chronic management reward implementation, post discharge and care management home visits.

Do I lose my supplemental benefits?

No.

Does this change my TRICARE?

No.

Do I have to give up my Medicare Card?

No, you get to keep your Medicare Card.

Why should I want to be enrolled with ilumed/ACO REACH?

You are not enrolled. We are letting you know that your doctor is, nothing changes with you and your doctor. You continue to have the freedom to select your Medicare physician.

Do I need a referral to see my specialist?

No.

How does this impact my specialist co-pays?

Your specialist co-payments will remain the same.
