Case Management Extender

The Case Management Extender responsibilities focus on assisting the Care Team to support the beneficiary’s health care needs. Drive communication within the entire team to facilitate and organize appropriate patient education, resources, and alignment of care.

Essential Job Functions

  • Work in collaboration with Case Management to act as a liaison between the PCP office and the beneficiary
  • Place outbound call as directed by case management to schedule and verify timely follow up appointments are maintained
  • Assist with facilitation of hospital admission and discharge information to internal team and the beneficiaries PCP in a timely manner
  • Expedite all educational material to the beneficiaries on Case and Disease Management encourage enrollment into the program
  • Educate beneficiaries on program benefits and resources
  • Assist in updating and maintaining the beneficiary’s demographic information
  • Support patient satisfaction with care team and program benefits
  • Assist case management with aligning beneficiaries with community resources based on their needs
  • Other duties as requested

Knowledge, Skills and Competencies

  • Good organizational skills and ability to handle multiple priorities and deadlines
  • Must be able to prioritize and cope with stress in a positive manner
  • Ability to communicate effectively both verbally and in writing with beneficiary and care team
  • Must present a clean, professional appearance, well-groomed and wear clean clothing, free of holes and tears
  • Proficient in Microsoft Office and web-based applications
  • Solid verbal and written communication skills
  • Working knowledge Microsoft Office
  • Demonstrates flexibility in organizational needs to perform other duties as assigned

Education and Experience

  • High school diploma or equivalent
  • At least (2) two years of healthcare customer service experience preferred

Physical Requirements

  • Prolonged periods of sitting at a desk and working on a computer.
